Job Description

Liberty Healthcare Management is currently seeking a compassionate and experienced Hospice Registered Nurse (RN) to join our dedicated team in Harnett & Johnston Counties on a full-time basis. As a Hospice RN, you will apply nursing theories, provide quality care, and coordinate with patients, families, physicians, and the Hospice team ensuring the highest level of comfort and support.

Responsibilities

• Apply nursing knowledge and skills in caring for hospice patients.
• Document patient care accurately and ensure it meets reimbursement standards.
• Coordinate patient care with all stakeholders involved.
• Maintain effective communication with patients, families, and agency staff.
• Participate in interdisciplinary team meetings and quality reviews.

Requirements

• Active RN licensure in the state of practice.
• Minimum two years of clinical nursing experience with at least one year in hospice care preferred.
• Knowledge of home health and hospice care programs.
• Compliance with nursing practices, agency policies, and reimbursement guidelines.
• Current CPR certification.
• Willingness to travel, work weekends, evenings, and holidays as needed.
