Job Description
Liberty Hospice is currently seeking a dedicated Hospice Registered Nurse (RN) to join our team on a full-time basis in Harnett & Johnston Counties. As an RN at Liberty Hospice, you will apply nursing theories and concepts while providing compassionate care and maintaining documentation that meets reimbursement guidelines. You will collaborate with the patient, family, physician, and Hospice team to ensure the physical and emotional well-being of the patients.
Responsibilities
- Apply nursing theories and concepts.
- Coordinate care with patients, families, physicians, and the Hospice team.
- Communicate effectively with patients/families and agency personnel.
- Participate in IDT meetings and quality reviews.
Requirements
- Active RN licensure in the state of practice.
- 2+ years of clinical nursing experience and at least 1 year of hospice nursing experience preferred.
- Knowledge of home health and hospice care programs.
- Compliance with nursing practice standards and agency policies.
- CPR certification and willingness to accept travel and on-call assignments.
