Job Description
Join the Liberty Healthcare Management team as a full-time MDS Coordinator (RN License Required). As part of our dedicated staff, you will play a crucial role in maintaining accuracy and compliance in the MDS process, ensuring quality care for our residents.
Responsibilities
- Maintain a schedule for MDS due dates and coordinate completion by all disciplines.
- Review and input MDS data accurately, meeting state requirements.
- Formulate Resident Care Plans in collaboration with the care team.
- Conduct regular RCP meetings and ensure staff compliance with care plans.
- Collaborate with internal departments to ensure RCP understanding and compliance.
- Perform additional duties as assigned by supervisors.
Requirements
- Registered Nurse with valid RN license from the North Carolina Board of Nursing.
- Experience with MDS-RAP, Care Planning, RUG-IV, and MDS 3.0 preferred.
- Ability to adhere to facility policies, procedures, and regulatory requirements.
