Overview

Under the supervision of the MCC Program Manager and Nurse Manager, the Medical Care Manager (MCM) must be an RN and work in collaboration with MCC staff. The MCM conducts assessments, integrated care planning, and monitors patient’s progress in conjunction with the Patient Case Managers (PCM). The MCM is focused on meeting the needs of patients who are experiencing medical adherence issues, significant changes in HIV health status, or multiple health diagnoses affecting a patient’s HIV status.

The MCM is focused on coordinating patient care, providing patient education, treatment adherence, managing side effects, medical nutrition, co-infections, preventative care, medication adherence, and risk reduction. The role does not emphasize Clinical Nursing duties but requires strong clinical background, critical thinking skills, and competency for patient care.

Essential Functions and Responsibilities

• Collaborate with healthcare providers, MCC staff, and external organizations for patient care plans.
• Provide support to the Patient Care Manager and engage in outreach activities.
• Make independent decisions, oversee specialist appointments, and ensure patient communication.
• Develop and implement health education materials for clients and families.
• Assist with staff training, attend meetings, and ensure compliance with regulations.
• Triage patients when needed and perform other assigned duties.

Qualifications

• Valid RN license in California.
• 3+ years direct patient care experience, preferably in ambulatory care settings.
• Experience in HIV/AIDS, Hepatitis C, and chronic disease care preferred.
• Knowledge of infectious diseases and psychosocial needs impact on health.
• Ability to work with diverse populations in a multicultural setting.
