Registered Nurse (RN) - Home Health / Hospice

Job Description

American Home Health Care, a leading provider of home health and hospice services, is currently looking for a Full-time or Part-time Registered Nurse (RN) to join our team in Alameda County and Contra Costa County. The RN will be responsible for coordinating and directing patient care based on individual needs, utilizing advanced assessment and decision-making skills. As an essential member of our team, the RN will ensure proper referrals, interdisciplinary conferences, and thorough documentation, consistently applying knowledge and experience to new patient populations.

Our mission is to provide compassionate end-of-life care and grief support in hospice, and professional services to assist patients in achieving their highest potential in home health care.

Responsibilities

• Provide comprehensive assessment, planning, implementation, and evaluation for a caseload of home care clients.
• Maintain updated clinical records and documentation according to agency policies.
• Complete initial assessments, re-evaluate patient needs, and coordinate patient care.
• Participate in on-call duties, ensure necessary equipment and services, and supervise home health aide care.

Requirements

• Current RN License in the State of California.
• Minimum 1 year of recent professional nursing experience in public health, home care, or hospice.
• Current CPR certification and a valid driver's license with insurance.
