Job Description
We are looking for a compassionate and skilled Registered Nurse (RN) to join our hospice home visits team in Dublin, GA. As a Registered Nurse, you will have the opportunity to provide exceptional care to patients with life-limiting illnesses, work collaboratively with a dedicated team, and make a meaningful impact on the lives of those in need. Responsibilities
- Demonstrate excellence in customer service and maintain a positive professional image.
- Conduct comprehensive assessments of patients with life-limiting illnesses, considering various factors.
- Develop individualized care plans in collaboration with the interdisciplinary hospice team.
- Conduct regular home visits to assess patients' conditions and address emerging issues.
- Administer medications as prescribed, educate patients and families, and maintain detailed documentation.
Requirements
- Minimum of 12 months of hospice in-home experience in the last 3 years.
- Experience with HCHB.
- Valid BLS certification issued by the American Heart Association.
- Eligibility to work in the United States and an RN license in Georgia.
