Registered Nurse Transitions Care Manager (RN)

Job Description

We are looking for a dedicated and experienced Registered Nurse Transitions Care Manager (RN) to join LifeBridge Health in Baltimore, MD. As a Nurse Transitions Navigator, you will play a crucial role in assessing, educating, and providing resources for high-risk individuals in need of transitional care.

Responsibilities

• Identify high-risk individuals and collaborate with clinical teams.
• Manage the successful transition of patients from hospital to the community.
• Conduct patient visits, provide health education, and coordinate healthcare needs.
• Develop comprehensive transition plans based on clinical standards of care.

Requirements

• BSN and Maryland RN license required.
• Minimum 2+ years of experience in clinical settings.
• American Heart Association CPR Certification.
• Strong knowledge of healthcare transitions and patient education.
