Job Description

We are looking for a dedicated Registered Nurse Transitions Care Manager (RN) to join LifeBridge Health in Baltimore, MD. The Nurse Transitions Navigator plays a crucial role in supporting high-risk individuals in navigating the healthcare system, providing assessments, education, and necessary resources.

Responsibilities

• Identify and assist individuals at high risk for increased healthcare utilization.
• Manage successful transitions from hospital to the community, including conducting patient visits, providing health education, and coordinating healthcare needs.
• Develop tailored transition plans, utilizing technology and addressing barriers to care, to support patient self-management of chronic illnesses.

Qualifications

• Bachelors of Science in Nursing (BSN) and Maryland Registered Nurse License.
• 2+ years of relevant experience with strong technical/clinical skills.
• American Heart Association CPR Certification.

Physical Demands

• Light physical exertion, with a focus on mobility during work activities.
