Responsibilities
- Provide skilled nursing care to patients in their homes.
- Work with an interdisciplinary team to deliver comprehensive patient care.
- Collaborate with patients' providers to achieve positive outcomes.
- Maintain accurate patient documentation and records.
Minimum Qualifications
- Associate's Degree in Nursing (RN Required).
- Valid Driver's License.
- Registered Nurse License from Ohio Board of Nursing.
Work Schedule
Day Shift, 40 hours per week.
