Job Description:

Overview

As a Medical Care Manager (MCM) at Men's Health Foundation, the RN will collaborate with healthcare providers to plan, organize, and implement patient care plans for chronic diseases. The MCM ensures patients receive quality care, support, and benefits from educational resources.

Responsibilities:

• Collaborate with healthcare providers, MCC staff, and outside organizations to implement patient care plans.
• Provide support to the Patient Care Manager.
• Make independent and substantial decisions to benefit patient care.
• Develop materials for health education and support the coordination of specialty referrals.

Qualifications:

• Valid RN license in California.
• Minimum three years of direct patient care experience, preferably in ambulatory care.
• Experience in HIV/AIDS, Hepatitis C, and chronic disease care.
• Ability to work effectively in a multicultural environment.

Language Skills:
