SOCIAL WORKER

Job Description

We are looking for a dedicated Social Worker to join our team at LifeBridge Health. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients and their families, addressing various challenges such as adjustment issues, financial concerns, family relationships, and end-of-life counseling.

Responsibilities

• Conduct psychosocial assessments of patients and families.
• Provide counseling on a range of issues including institutional living, financial matters, and discharge planning.
• Collaborate with healthcare teams to develop comprehensive care plans.
• Support patients in coping with medical diagnoses and treatment.

Requirements

• Bachelor's in Social Work (BSW) required, Master's degree preferred.
• LCSW-C and LGSW certifications preferred.
• Experience in a healthcare setting is a plus.
• Excellent communication and interpersonal skills.
