Job Description
We are looking for a compassionate and dedicated Social Worker to join our team at LifeBridge Health. As a Social Worker, you will provide essential clinical social work services and psychosocial assessments to patients and their families, addressing various challenges they may face in the healthcare setting. This full-time position offers the opportunity to make a significant impact on the well-being of individuals and families.
Responsibilities
- Conduct psychosocial assessments and provide counseling to patients and families.
- Assist with adjustment to institutional living, discharge planning, and end-of-life care.
- Address financial concerns, family relationships, and other social issues affecting patient care.
Requirements
- Bachelor's in Social Work (BSW) required; Master's degree preferred.
- Licensed Certified Social Worker-Clinical (LCSW-C) preferred.
- Licensed Graduate Social Worker (LGSW) certification.
