Job Description
We are looking for a dedicated Social Worker to join our team at LifeBridge Health. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients, addressing various challenges they may face during their stay at Levindale Hospital in Baltimore, MD.
Responsibilities
- Provide counseling and support to patients and their families on a range of issues including institutional living adjustment, financial concerns, family relationships, and end-of-life issues.
- Assist with discharge planning and address challenges related to medical conditions and aging.
- Collaborate with healthcare teams to ensure comprehensive care and support for patients.
Requirements
- Bachelor's degree in Social Work (BSW) required, Master's degree preferred.
- Licensed Certified Social Worker - Clinical (LCSW-C) preferred.
- Licensed Graduate Social Worker (LGSW) certification.
