Job Description
We are looking for a compassionate and dedicated Social Worker to join LifeBridge Health's team at Levindale Hospital in Baltimore, MD. As a full-time Social Worker, you will provide clinical social work services to patients and their families, addressing various challenges such as adjustment to institutional living, financial concerns, family relationships, and discharge planning.
Responsibilities
- Conduct psychosocial assessments and provide counseling to patients/residents and families.
- Assist individuals in coping with medical diagnoses, aging, and end-of-life issues.
- Collaborate with healthcare professionals to ensure holistic patient care.
- Participate in discharge planning and support patients through transitions.
Requirements
- Bachelor's in Social Work (BSW) required; Master's degree preferred.
- Licensed Certified Social Worker-Clinical (LCSW-C) preferred.
- Licensed Graduate Social Worker (LGSW) certification.
