Job Description
We are looking for a dedicated and compassionate Social Worker to join the dynamic team at LifeBridge Health. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients/residents and families, addressing a range of common challenges related to institutional living, family dynamics, medical procedures, aging, discharge planning, and counseling for end-of-life issues.
Responsibilities
- Provide clinical social work services to patients/residents and families.
- Assess and address issues such as adjustment to institutional living, financial concerns, and family relationships.
- Offer counseling on understanding medical procedures and treatments.
- Support individuals dealing with changes related to aging and end-of-life issues.
Requirements
- Bachelor's degree in Social Work (BSW); Master's degree preferred.
- LCSW-C preferred, LGSW required.
- Prior experience in a similar role is beneficial.
- Commitment to providing compassionate care and support to individuals and families.
