Job Description
We are looking for a compassionate Social Worker to join LifeBridge Health, a leading healthcare provider in Maryland. In this full-time position at Levindale Hospital in Baltimore, you will provide clinical social work services to patients/residents and families. Your responsibilities will include conducting psychosocial assessments, addressing various issues such as adjustment to institutional living, financial concerns, family relationships, and counseling related to health challenges and end-of-life care.
Responsibilities
- Conduct psychosocial assessments and provide clinical social work services.
- Assist patients/residents and families with various concerns and challenges.
- Engage in discharge planning and end-of-life counseling.
Requirements
- Bachelor's in Social Work (BSW) required, Master's degree preferred.
- Licensed Certified Social Worker-Clinical (LCSW-C) preferred.
- Licensed Graduate Social Worker (LGSW) certification.
