Job Description
We are looking for a passionate and dedicated Social Worker to join our team at LifeBridge Health. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients and their families, addressing various challenges such as adjustment to institutional living, financial concerns, family relationships, and end-of-life counseling.
Responsibilities
- Deliver clinical social work services to patients/residents and families.
- Conduct psychosocial assessments and provide counseling on various issues.
- Assist in discharge planning and coping with changes related to aging.
- Collaborate with healthcare teams to ensure comprehensive care.
Requirements
- Bachelor's in Social Work (BSW), Master's degree preferred.
- LCSW-C preferred, LGSW certification required.
- Prior experience in a healthcare setting is a plus.
- Excellent communication and interpersonal skills.
