Job Description

We are looking for a dedicated Social Worker to join our compassionate team at LifeBridge Health. In this full-time position based in Baltimore, MD at Levindale Hospital, you will have the opportunity to provide invaluable psychosocial assessments and clinical social work services to patients, residents, and their families. The role encompasses addressing various challenges like adjustment to institutional living, financial concerns, family relationships, and counseling related to medical issues, aging, and end-of-life care planning.

Responsibilities

• Conduct psychosocial assessments.
• Provide counseling and support to patients and families.
• Participate in discharge planning.
• Collaborate with healthcare professionals to ensure comprehensive care.

Requirements

• Bachelor's in Social Work (BSW), Master's degree preferred.
• LCSW-C preferred.
• LGSW certification.
