SOCIAL WORKER

Job Description

We are looking for a compassionate and dedicated Social Worker to join our team at LifeBridge Health. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients/residents and their families, addressing various challenges such as adjustment to institutional living, financial concerns, family relationships, and counseling related to medical procedures and end-of-life care.

Responsibilities

• Provide clinical social work services and psychosocial assessments to patients/residents and families.
• Address common issues including adjustment to institutional living, financial concerns, and family relationships.
• Offer counseling related to medical procedures, aging, discharge planning, and end-of-life care.

Requirements

• Bachelor's in Social Work (BSW) required; Master's degree preferred.
• LCSW-C preferred.
• LGSW certification.
