SOCIAL WORKER

Job Description

We are currently looking for a dedicated Social Worker to join our team at LifeBridge Health in Baltimore, MD. As a Social Worker, you will provide essential clinical social work services and psychosocial assessments to patients and their families. You will address a variety of common challenges such as adjustment to institutional living, financial concerns, family relationships, aging-related changes, discharge planning, and grief counseling.

Responsibilities

• Deliver clinical social work services to patients and families.
• Conduct psychosocial assessments and assist in resolving common issues.
• Coordinate with interdisciplinary teams for comprehensive patient care.
• Provide counseling and support related to complex medical situations.

Requirements

• Bachelor's in Social Work (BSW) is required.
• Master's degree in Social Work preferred.
• Licensed Certified Social Worker-Clinical (LCSW-C) preferred.
• Licensed Graduate Social Worker (LGSW) certification.
