Job Description
LifeBridge Health is currently seeking a compassionate and dedicated Social Worker to join our team at Levindale Hospital in Baltimore, MD. As a full-time Social Worker, you will provide clinical social work services to patients and families, supporting them through various challenges and transitions.
Responsibilities
- Conduct psychosocial assessments and provide support for common issues such as adjustment to institutional living, financial concerns, and family relationships.
- Offer counseling on medical diagnoses, treatments, aging, discharge planning, and end-of-life care.
- Collaborate with healthcare teams to ensure comprehensive care for patients/residents.
Requirements
- Bachelor's in Social Work (BSW) required, Master's degree preferred.
- LCSW-C preferred, LGSW certification.
- Prior experience in a healthcare setting is advantageous.
