Job Description
We are looking for a compassionate Social Worker to join LifeBridge Health, a leading health care provider in Maryland. As a full-time member of the team based at Levindale Hospital in Baltimore, MD, you will provide clinical social work services to patients and families, addressing various challenges such as institutional living adjustment, financial concerns, and end-of-life counseling.
Responsibilities
- Conduct psychosocial assessments and provide counseling.
- Assist with discharge planning and end-of-life support.
- Collaborate with interdisciplinary teams to address patient needs.
- Advocate for patients' rights and access to resources.
Requirements
- Bachelor's in Social Work (BSW); Master's degree preferred.
- LCSW-C certification is preferred.
- LGSW certification is required.
- Prior experience in a clinical social work setting is advantageous.
