Job Description
We are looking for a dedicated Social Worker to join our team at LifeBridge Health. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients/residents and their families. Your responsibilities will include addressing various challenges such as adjustment to institutional living, financial concerns, family relationships, discharge planning, and counseling on end-of-life issues.
Responsibilities
- Provide clinical social work services and psychosocial assessments.
- Address common patient and family challenges.
- Assist with discharge planning and end-of-life counseling.
Requirements
- Bachelor's in Social Work (BSW) required; Master's degree preferred.
- LCSW-C preferred; LGSW certification also beneficial.
