Job Description
We are currently seeking a dedicated Social Worker to join our compassionate team at LifeBridge Health. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients, residents, and their families. You will address various concerns such as adjustment to institutional living, financial issues, family relationships, healthcare understanding, aging-related changes, discharge planning, and counseling in end-of-life situations.
Responsibilities
- Deliver clinical social work services and psychosocial assessments.
- Support patients, residents, and families in coping with medical diagnoses and treatments.
- Assist in discharge planning and end-of-life counseling.
- Collaborate with healthcare teams to provide comprehensive care.
Requirements
- Bachelor's in Social Work (BSW) required.
- Master's degree in Social Work preferred.
- Licensed Certified Social Worker-Clinical (LCSW-C) preferred.
- Licensed Graduate Social Worker (LGSW) certification.
