Job Description
We are looking for an empathetic and dedicated Social Worker to join LifeBridge Health in Baltimore, MD. As a key member of our team, you will provide essential clinical social work services and psychosocial assessments to patients and their families, addressing a wide range of common challenges.
Responsibilities
- Offer support and guidance to patients and families facing various social and emotional issues.
- Conduct psychosocial assessments to determine patients' needs and appropriate interventions.
- Assist with discharge planning and bereavement counseling.
- Collaborate with healthcare professionals to ensure comprehensive care.
Requirements
- Bachelor's in Social Work (BSW) required; Master's degree preferred.
- Licensed Certified Social Worker-Clinical (LCSW-C) preferred.
- Licensed Graduate Social Worker (LGSW) certification.
