Job Description
We are looking for a passionate Social Worker to join our dedicated team at LifeBridge Health. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients and their families, addressing a variety of challenges such as adjustment to institutional living, financial concerns, family relationships, and more. You will play a crucial role in discharge planning and counseling related to death and dying.
Responsibilities
- Provide clinical social work services and psychosocial assessments.
- Address common problems such as adjustment to institutional living and financial concerns.
- Assist patients and families with understanding medical procedures and treatments.
- Participate in discharge planning and provide counseling on end-of-life matters.
Requirements
- Bachelor's in Social Work (BSW) required; Master's degree preferred.
- LCSW-C certification preferred.
- LGSW certification required.
