Job Description
We are looking for a compassionate and dedicated Social Worker to join LifeBridge Health at Levindale Hospital in Baltimore, MD. As a valuable member of our Allied Health team, you will provide clinical social work services and psychosocial assessments to patients, focusing on a range of common issues such as adjustment to institutional living, financial concerns, family relationships, aging-related changes, discharge planning, and end-of-life counseling.
Responsibilities
- Conduct psychosocial assessments and provide clinical social work support.
- Help patients and families navigate medical procedures, treatments, and end-of-life decisions.
- Assist with discharge planning and offer counseling related to various emotional and social challenges.
Requirements
- Bachelor's in Social Work (BSW) required; Master's degree strongly preferred.
- Licensed Certified Social Worker-Clinical (LCSW-C) preferred.
- Licensed Graduate Social Worker (LGSW) certification.
