Job Description
We are looking for a compassionate Social Worker to join LifeBridge Health. As a vital member of our team, you will provide clinical social work services and psychosocial assessments to patients, addressing various challenges they may face. Responsibilities
- Offer counseling and support to patients and families on a range of issues such as adjustment to institutional living, financial concerns, and end-of-life planning.
- Conduct psychosocial assessments to identify needs and develop appropriate care plans.
- Collaborate with healthcare teams to ensure comprehensive care for patients.
- Participate in discharge planning and provide emotional support during difficult times.
Requirements
- Bachelor's degree in Social Work (BSW); Master's degree preferred.
- Licensed Certified Social Worker-Clinical (LCSW-C) designation is preferred.
- Licensed Graduate Social Worker (LGSW) credential.
- Strong communication and interpersonal skills.
