Job Description
We are looking for a dedicated Social Worker to join our team at LifeBridge Health. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients, addressing various common problems such as adjustment to institutional living, financial concerns, family relationships, and counseling related to death and dying.
Responsibilities
- Deliver clinical social work services to patients and families.
- Perform psychosocial assessments to address common challenges.
- Assist with adjustment to institutional living and financial concerns.
- Provide counseling related to medical diagnoses, aging, discharge planning, and end-of-life care.
Requirements
- Bachelor's in Social Work (BSW) required; Master's degree preferred.
- LCSW-C certification preferred.
- LGSW certification required.
