Job Description
We are looking for a compassionate Social Worker to join our team at LifeBridge Health. In this full-time role based in Baltimore, MD, at Levindale Hospital, you will provide clinical social work services and psychosocial assessments to patients and families. Your responsibilities will include addressing a range of issues such as adjustment to institutional living, financial concerns, family relationships, and counseling related to death and dying.
Key Responsibilities
- Conduct psychosocial assessments and provide clinical social work services.
- Assist patients and families with various common challenges.
- Facilitate discharge planning and provide counseling.
Requirements
- Bachelor's in Social Work (BSW) required; Master's degree preferred.
- LCSW-C preferred; LGSW certification.
