SOCIAL WORKER

Job Description

We are looking for a compassionate and dedicated Social Worker to join our team at LifeBridge Health. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients and their families, addressing a variety of challenges they may face. This role involves supporting individuals in coping with institutional living, financial issues, family dynamics, medical diagnoses, aging-related changes, discharge planning, and end-of-life counseling.

Responsibilities

• Offer clinical social work services to patients and families.
• Conduct psychosocial assessments and provide support for common issues.
• Counsel patients on various aspects related to their well-being.
• Develop comprehensive discharge plans.

Requirements

• Bachelor's degree in Social Work (BSW).
• Master's degree in Social Work preferred.
• LCSW-C certification preferred.
• LGSW certification required.
