Job Description
We are looking for a dedicated Social Worker to join our team at LifeBridge Health. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients/residents and families, addressing various challenges they may face. Join us in making a meaningful impact on the lives of our patients and their families.
Responsibilities
- Provide clinical social work services and assessments to address adjustment issues, financial concerns, family dynamics, and more.
- Offer counseling and support related to medical procedures, aging, discharge planning, and end-of-life care.
Requirements
- Bachelor's in Social Work (BSW) required; Master's degree preferred.
- Licensed Certified Social Worker-Clinical (LCSW-C) preferred.
- Licensed Graduate Social Worker (LGSW) certification.
