Job Description
We are looking for a compassionate and dedicated Social Worker to join our team at LifeBridge Health. As a Social Worker, you will provide clinical social work services to patients/residents and families, addressing a range of common issues such as adjustment to institutional living, financial concerns, family relationships, and discharge planning.
Responsibilities
- Conduct psychosocial assessments and provide counseling to patients and families.
- Address issues related to aging, powerlessness, and end-of-life care.
- Collaborate with healthcare teams to develop comprehensive care plans.
- Participate in discharge planning and ensure smooth transitions for patients.
Requirements
- Bachelor's in Social Work (BSW) required; Master's degree preferred.
- Licensed Certified Social Worker (LCSW-C) preferred.
- Licensed Graduate Social Worker (LGSW) certification.
