Job Description

We are looking for a dedicated Social Worker to join our team at LifeBridge Health. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients, addressing various challenges such as adjustment to institutional living, financial concerns, family relationships, and more. Responsibilities

• Provide clinical social work services to patients and families.
• Conduct psychosocial assessments and address common problems patients face.
• Participate in discharge planning and provide counseling related to death and dying.

Requirements

• Bachelor's in Social Work (BSW); Master's degree preferred.
• LCSW-C certification preferred.
• LGSW certification.
