SOCIAL WORKER

Job Description

We are looking for a compassionate Social Worker to join our team at LifeBridge Health. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients and their families, addressing a variety of common challenges related to institutional living, financial issues, family dynamics, medical understanding, aging, and end-of-life planning.

Responsibilities

• Conduct psychosocial assessments of patients and families.
• Provide counseling on various personal and medical issues.
• Assist with discharge planning and end-of-life support.
• Collaborate with healthcare teams to advocate for patients' needs.

Requirements

• Bachelor's in Social Work (BSW) required; Master's degree preferred.
• LCSW-C certification preferred.
• LGSW certification.
• Strong communication and interpersonal skills.

Preferred Skills

• Experience in clinical social work.
• Knowledge of medical terminologies.
• Ability to work in a fast-paced healthcare environment.
