Job Description

We are looking for a dedicated Social Worker to join LifeBridge Health in Baltimore, MD. As a member of our team, you will provide clinical social work services to patients and their families, addressing various challenges such as adjustment to institutional living, financial concerns, and discharge planning. This full-time position offers a day shift from 8:00am to 4:30pm.

Responsibilities

  • Deliver psychosocial assessments and counseling to address patient and family needs.
  • Assist with discharge planning and end-of-life support.
  • Work collaboratively with healthcare professionals to ensure comprehensive care.

Requirements

  • Bachelor's in Social Work (BSW) required; Master's degree preferred.
  • Licensed Certified Social Worker-Clinical (LCSW-C) preferred.
  • Licensed Graduate Social Worker (LGSW) certification.
