Job Description
We are looking to hire a dedicated Social Worker to join our compassionate team at LifeBridge Health. As a Social Worker, you will provide essential clinical social work services and psychosocial assessments to patients, residents, and their families, addressing various challenges they may encounter during their healthcare journey.
Responsibilities
- Offer clinical social work services to address a range of issues such as adjustment to institutional living, financial concerns, family relationships, and end-of-life counseling.
- Collaborate with patients, families, and healthcare professionals to create effective care plans.
- Assist with discharge planning and support patients in navigating complex medical procedures and treatments.
Requirements
- Bachelor's degree in Social Work (BSW) is required; Master's degree is preferred.
- LCSW-C certification is preferred.
- LGSW certification is required.
