Job Description

We are looking for a compassionate and dedicated Social Worker to join LifeBridge Health's team at Levindale Hospital in Baltimore, MD. As a full-time Social Worker, you will provide clinical social work services and psychosocial assessments to patients/residents and families, addressing a range of common concerns including adjustment to institutional living, financial issues, family dynamics, medical procedures, aging, and end-of-life planning.

Responsibilities

• Provide clinical social work services and psychosocial assessments.
• Address various patient and family concerns with empathy and professionalism.
• Assist with discharge planning and counseling related to death and dying.
• Collaborate with healthcare teams to support patients' well-being.

Requirements

• Bachelor's in Social Work (BSW).
• Master's degree preferred.
• LCSW-C preferred.
• LGSW certification.
