Job Description
We have an exciting opportunity for a full-time Social Worker at LifeBridge Health in Baltimore, MD. As part of our dynamic team, you will provide clinical social work services to patients/residents and families, addressing a variety of common challenges such as adjustment to institutional living, financial concerns, family relationships, and more.
Responsibilities
- Conduct psychosocial assessments and provide counseling to address patient and family needs.
- Assist with discharge planning and end-of-life counseling.
- Collaborate with healthcare professionals to ensure comprehensive care.
Requirements
- Bachelor's in Social Work (BSW) required; Master's degree preferred.
- LCSW-C certification is preferred.
- Strong communication and interpersonal skills.
- Ability to work effectively in a fast-paced healthcare environment.
