Job Description
We are looking for a compassionate and dedicated Social Worker to join LifeBridge Health in Baltimore, MD at Levindale Hospital. As a full-time member of our Allied Health team, you will provide clinical social work services and psychosocial assessments to patients, residents, and families, addressing a range of common issues to support their well-being.
Job Responsibilities
- Conduct psychosocial assessments and provide counseling to address various challenges such as adjustment to institutional living, financial concerns, family relationships, and end-of-life care.
- Assist individuals in understanding and coping with medical diagnoses, treatments, and aging-related changes.
- Participate in discharge planning and collaborate with healthcare team members to ensure comprehensive care.
Requirements
- Bachelor's degree in Social Work (BSW) required; Master's degree preferred.
- LCSW-C preferred.
- LGSW certification.
