Job Description
We are looking for a compassionate and dedicated Social Worker to join LifeBridge Health's team at Levindale Hospital in Baltimore, MD. As a full-time Social Worker, you will be responsible for providing clinical social work services and psychosocial assessments to patients, residents, and their families. Your role will involve addressing various challenges such as adjustment to institutional living, financial concerns, family relationships, aging-related changes, discharge planning, and counseling on end-of-life issues.
Responsibilities
- Conduct psychosocial assessments and provide counseling.
- Assist patients and families in coping with medical conditions and procedures.
- Develop and implement discharge plans.
- Collaborate with healthcare professionals to ensure comprehensive care.
Requirements
- Bachelor's in Social Work (BSW); Master's degree preferred.
- LCSW-C preferred; LGSW certification required.
- Ability to demonstrate empathy, communication skills, and cultural competence.
- Experience in a healthcare or hospital setting is a plus.
