Job Description
We are looking for a compassionate and dedicated Social Worker to join our team at LifeBridge Health. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients, residents, and families, addressing various challenges they may face. Responsibilities
- Conduct psychosocial assessments and provide counseling to individuals and families.
- Address issues related to adjustment, financial concerns, family relationships, aging, and end-of-life care.
- Collaborate with healthcare professionals to develop and implement care plans.
- Participate in discharge planning processes.
Requirements
- Bachelor's in Social Work (BSW) required; Master's degree preferred.
- Licensed Certified Social Worker-Clinical (LCSW-C) preferred.
- Licensed Graduate Social Worker (LGSW) certification.
- Strong communication and interpersonal skills.
