Job Description
We are looking for a compassionate and dedicated Social Worker to join our team at LifeBridge Health. As a valued member of our allied health professionals, you will provide clinical social work services and psychosocial assessments to patients, addressing various challenges they may face.
Responsibilities
- Offer support and counseling to patients and families on adjustment to institutional living, financial concerns, family relationships, and other social issues.
- Assist with discharge planning and provide counseling on end-of-life matters.
- Collaborate with healthcare teams to ensure the well-being of patients in our care.
Requirements
- Bachelor's in Social Work (BSW) required; Master's degree is preferred.
- Licensed Certified Social Worker-Clinical (LCSW-C) preferred.
- Licensed Graduate Social Worker (LGSW) certification.
