Job Description
We are looking for a compassionate and dedicated Social Worker to join LifeBridge Health at Levindale Hospital in Baltimore, MD. As a Social Worker, you will provide essential clinical social work services and psychosocial assessments to patients, residents, and their families, addressing various challenges such as adjustment to institutional living, financial concerns, family relationships, and counseling related to death and dying.
Responsibilities
- Offer psychosocial assessments to patients and families.
- Provide counseling on a range of issues, including adjustment difficulties and end-of-life care.
- Participate in discharge planning processes.
Requirements
- Bachelor's in Social Work (BSW); Master's degree preferred.
- LCSW-C preferred; LGSW certification required.
