Job Description
We are looking for a dedicated Social Worker to join our compassionate team at LifeBridge Health. As a Social Worker, you will provide clinical services and psychosocial assessments to patients, residents, and families, addressing various challenges they may face. This is a full-time position based in Baltimore, MD at Levindale Hospital.
Responsibilities
- Conduct psychosocial assessments and provide counseling to individuals and families.
- Assist with discharge planning and end-of-life care.
- Address issues such as adjustment to institutional living, financial concerns, and family relationships.
- Collaborate with healthcare teams to provide holistic care.
Requirements
- Bachelor's degree in Social Work (BSW); Master's degree preferred.
- Licensed Certified Social Worker-Clinical (LCSW-C) preferred.
- Licensed Graduate Social Worker (LGSW) certification.
