Job Description
We are looking for a dedicated Social Worker to join our team at LifeBridge Health. As a Social Worker, you will be providing clinical social work services and psychosocial assessments to patients and their families, addressing various challenges they may face related to institutional living, financial concerns, family dynamics, medical treatments, aging, discharge planning, and counseling for end-of-life situations.
Responsibilities
- Conduct psychosocial assessments and provide clinical social work services.
- Address a wide range of patient and family issues related to healthcare and well-being.
- Collaborate with interdisciplinary teams to ensure comprehensive care for patients.
- Participate in discharge planning and counseling for patients and families.
Requirements
- Bachelor's in Social Work (BSW) required; Master's degree preferred.
- LCSW-C preferred; LGSW certification required.
- Prior experience in a healthcare setting is advantageous.
- Strong communication, empathy, and interpersonal skills.
