Job Description
We are looking for a dedicated Social Worker to join our compassionate team at LifeBridge Health. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients, assisting them with various challenges they may face. This role involves helping individuals cope with institutional living, financial issues, family dynamics, and emotional difficulties related to their health.
Responsibilities
- Conduct psychosocial assessments of patients/residents and their families.
- Assist individuals in adjusting to their current living situation and health conditions.
- Provide counseling and support related to aging, end-of-life care, and discharge planning.
- Collaborate with healthcare professionals to ensure holistic patient care.
Requirements
- Bachelor's in Social Work (BSW) required; Master's degree preferred.
- Licensed Certified Social Worker - Clinical (LCSW-C) preferred.
- Licensed Graduate Social Worker (LGSW) certification.
- Strong communication and interpersonal skills.
