Job Description

We are seeking a passionate Social Worker to join our team at LifeBridge Health. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients/residents and their families. Your responsibilities will include addressing various social and psychological challenges such as adjustment to institutional living, financial concerns, family relationships, and end-of-life counseling.

Responsibilities

• Provide clinical social work services and psychosocial assessments.
• Address common issues related to patient and family well-being.
• Conduct counseling for end-of-life care and emotional support.
• Collaborate with healthcare professionals to ensure comprehensive patient care.

Requirements

• Bachelor's in Social Work required; Master's degree preferred.
• LCSW-C certification is preferred.
• LGSW certification.
