SOCIAL WORKER

Job Description

We are looking for a compassionate and skilled Social Worker to join our team at LifeBridge Health. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients/residents and families, addressing various challenges such as adjustment to institutional living, financial concerns, family relationships, and end-of-life care.

Responsibilities

• Offer clinical social work services and psychosocial assessments.
• Address patients' and families' concerns related to medical conditions and aging.
• Facilitate discharge planning and counseling on death and dying.
• Collaborate with healthcare teams to provide holistic care.

Requirements

• Bachelor's in Social Work (BSW); Master's degree preferred.
• LCSW-C preferred; LGSW credentials.
• Experience in a healthcare setting is a plus.
• Strong communication and interpersonal skills.
