SOCIAL WORKER

Job Description

We are looking for a dedicated Social Worker to join LifeBridge Health, one of Maryland's largest healthcare providers. As a Full-time Social Worker at Levindale Hospital in Baltimore, you will provide clinical social work services and psychosocial assessments to patients/residents and families, addressing various challenges such as adjustment to institutional living, financial concerns, family dynamics, aging-related changes, discharge planning, and end-of-life counseling.

Responsibilities

• Offer clinical social work services and conduct psychosocial assessments.
• Assist patients and families in coping with medical diagnoses, institutional living, financial issues, and end-of-life concerns.
• Collaborate with healthcare teams to develop and implement care plans.
• Provide counseling and support for patients and families during challenging times.

Requirements

• Bachelor's in Social Work (BSW); Master's degree is preferred.
• LCSW-C certification is preferred.
• LGSW certification.
• Previous experience in clinical social work.
